If you need to make a claim, please notify us as quickly as possible.  Call us toll free on 0800 COMBINED (266 246) 9am to 6.30 pm Monday to Friday.  We can provide advice over the phone and post out anything you may require to help action your claim.


To speed up the process you can download a Claim Form from this site rather than waiting for a claim form from us by mail.

IMPORTANT: Once downloaded, the form should be completed in full by you and your doctor and returned to us with your signature. On receipt of the completed form, we will review and assess your claim and policy coverage. We also advise you to carefully read the information on the claim form before having it completed.
